I haven't run into this yet, but I'm gonna guess one or a combination of two things. Replace the cutter (turn it 120 degrees for a new cutting edge), and/or go slower on the upstroke.

What is the inside diameter of the necks before seating? As you mentioned, if there is insufficient chamfer on the case necks they can grab and/or dig into the bullet itself.

Well that's a first for me...I've never heard of a gas port being off that much. I've heard of M1A's being off a little when you shim up the gas system, but never an AR from the get-go. Weird. Kudos...
