How concentric should the center holes be in a shaft before it is placed between centers to be turned? I'm guessing, the closer the better, or doesn't it really matter as long as they are fairly...

Why must the live center run true in the headstock spindle when turning between centers on the lathe?

quick question

A headstock live center or tailstock rovolving center which is running out when a shaft is turned all at once won't affect the shaft itself. Therefore the shaft will still be straight and concentric...

how do you use a steady rest?

After the shaft is in the machine and running true at the chuck, how should I indicate at the steady rest end? I know I can clamp an indicator to the shaft itself and sweep around the tailstock...

the little point left in the middle of the workpiece is because your tool is not on center, probably a little low. Shim the tool up until the nub is removed.
